M*Plus - MMA Medicaid Benefits Only with UnitedHealthcare

Health & Home Connection - Long Term Care (LTC) Benefits Only with UnitedHealthcare

M*Plus Connection - Both MMA and LTC Benefits with UnitedHealthcare
